Please list, in order of preference, the pet's name(s) whom you are applying for. Be as specific as possible. Please note, that filling out this application does not guarantee that your application will be the first received for this pet, nor does it give you the right to adopt this specific pet for certain. If you are not applying for a specific pet please specify this.
If you rent, please provide your landlords name and phone number. We may contact them to verify you may own a pet. Please also note any restrictions.
NOTE: Please bring a copy of your lease or mortgage statement when you come to adopt.
Do you plan to have this adopted pet neutered?
Do you own or rent your home?
If you currently own a dog or cat, please provide your veterinarian's name and phone number:
NOTE: Please ensure your existing pets are up to date on vaccines required for boarding before adopting a new pet.
Are there any allergies in your family that would be affected by a new pet?
Most puppies are born with intestinal parasites such as roundworms or hookworms. Ideally a puppy should be dewormed starting at 2 weeks of age, and again every two weeks through the 10th week of its life. Adult pets, and older puppies, should be kept on monthly heartworm prevention which doubles as an intestinal parasite prevention method. All WWR canines (puppies and adults) are started on deworming medication. Because it can take several rounds of this medication to fully rid a pet of parasites, your new puppy or dog may come to you needing additional medication. We recommend bringing a fecal sample to your first vet appointment with you, to see what specifically your vet would recommend. Are you willing to continue your newly adopted pet on an immediate and lifelong parasite TREATMENT and PREVENTION program?
Wright-Way Rescue recommends bringing your newly adopted pet for a health check-up within 72 hours of adoption. Abell Animal Hospital offers this initial check up free of charge to adopters. You may choose to use Abell or any other vet of your choice. Will you agree to bring your new pet in for a check-up immediately after you adopt (within 72 hours)?
Our application process enables us to ask our adopters about their previous pet experience and details regarding the type of home they are able to provide a homeless pet in need.
Please answer honestly as these questions allow us to appropriately match you with a suitable pet.  Applicants must be 21 years or older on the date of application.
The adoption donation is non-refundable in all cases including situations where allergies in the home are found. Please check yes if you understand this policy.
Wright-Way Rescue quarantines all of its adoptable pets prior to offering them for adoption. Because they come from "high risk" situations and we do not have control over their previous exposure to other pets and the environment, we cannot guarantee them. All medical bills are the adopter’s sole responsibility post-adoption and WWR is not obligated or able to pay any vet bills. Please check yes if you understand and agree to this policy.
I understand that filling out this survey/application does not guarantee that I will be approved for adoption. I also understand that the majority of pets offered to adoption at Wright-Way Rescue are mixed breed and that their exact breed mixture is not able to be guaranteed.
I certify that to the best of my ability, the above information is accurate and true. I realize that providing false information will disqualify me from adopting a pet through Wright-Way Rescue. Please type first and last name.
